About the position
The Schedule Specialist (LPN) at Feliciana Home Health is responsible for managing patient referrals and visit schedules, ensuring efficient service delivery through an automated scheduling system. This role emphasizes the importance of communication and organization in coordinating care for patients and supporting field staff in addressing scheduling issues.
Responsibilities
- Manage patient referrals and visit schedules.
- Utilize an automated scheduling system to maintain a calendar of services for episodic and per visit customers.
- Process workflow for scheduled, missed, rescheduled, reassigned, declined, and delivered visits.
- Monitor pending referrals daily and assign licensed professionals and case managers for start of care visits.
- Communicate daily with field staff regarding visits that are late, pending, or incomplete for resolution.
Requirements
- High school education or equivalent.
- Minimum one year of scheduling experience in a healthcare setting using an online scheduling system is preferred.
- Exceptional organizational skills.
- Strong customer service skills.
- Effective communication skills.
- Good decision-making skills.
- Working knowledge of state and federal regulations governing OASIS visits, supervisory, and reassessment visits.
