About the position

The Social Services Assistant plays a crucial role in supporting the Social Services Director at a skilled nursing facility. This position focuses on enhancing the quality of life and overall well-being of residents by providing high-quality care and services. The assistant is involved in planning, developing, organizing, implementing, and evaluating social services programs for residents and their legal representatives.

Responsibilities

  • Assist with planning and implementing a comprehensive Social Services program.
  • Provide counseling and support services for customers and their legal representatives.
  • Identify medical-related social needs of customers and provide appropriate services.
  • Maintain records relating to customers' social work needs and care.
  • Consult with the Director of Social Services and other department heads on interdisciplinary issues.
  • Work directly with customers and legal representatives experiencing personal and environmental difficulties.

Requirements

  • Bachelor's degree in social work is preferred; a degree in human services may also be considered.
  • Previous experience in the field is required.
  • Previous long-term care experience is strongly preferred.
  • Possess any Certifications/Licensures required by State regulations.
