The Social Worker serves as a Readjustment Counselor at the U.S. Department of Veterans Affairs, providing essential counseling services, outreach, and care coordination for veterans and their families. This role involves working within a multi-disciplinary team to support the readjustment of eligible individuals through therapeutic relationships, assessments, and individualized service plans.

  • Provide counseling services to eligible individuals, couples, and family members.
  • Conduct screening and assessment to establish therapeutic relationships and assess risk for self-harm.
  • Develop and update individualized readjustment counseling service plans with achievable goals.
  • Implement counseling plans through individual, group, couple, and family sessions.
  • Apply risk assessment procedures and develop safety plans for high-risk individuals.
  • Coordinate care with community resources and internal VA services to support veterans' readjustment.
  • Engage in outreach activities to connect with veterans and their families in the community.
  • Participate in team meetings for staff development and case discussions.
  • Document clinical interactions and provide consultation to other staff.
  • Must be a U.S. Citizen.
  • Master's degree in social work from an accredited school.
  • State licensure or certification to practice social work at the master's level.
  • Minimum of one year of post-MSW experience in health care or social work settings for GS-11 level.
  • Demonstrated knowledge of community resources and ability to coordinate services.
  • Skill in conducting psychosocial assessments and treatment interventions.
  • Knowledge of medical and mental health diagnoses and treatment procedures.
  • Experience working with veterans or military families.
  • Familiarity with VA benefits and services.
  • Ability to work in a multi-disciplinary team environment.
