About The Position

The Spiritual Care Counselor is a full-time professional role within the Hospice interdisciplinary team, responsible for developing and implementing care plans that provide spiritual care and support to patients and their families. This position requires collaboration with various stakeholders to ensure culturally sensitive spiritual care, documentation of spiritual needs, and provision of end-of-life information. The role also includes supporting hospice staff and participating in performance improvement activities.

Requirements

  • MDiv, Master of Pastoral Care, or Master of Theological Studies from an accredited university.
  • Completion of Clinical Pastoral Education residency preferred or equivalent experience.
  • Minimum one-year experience in a health-related field; hospice experience preferred.
  • Ability to use a computer and learn new software programs.
  • Reliable transportation to travel to patient/family homes and facilities.
  • Ability to lift and/or carry items up to 25 pounds.
  • Sensory abilities to complete physical assessments and communicate effectively with patients, families, and care providers.

Responsibilities

  • Provide immediate and long-term assessment and documentation of the spiritual situation and needs of patients and families.
  • Collaborate with patients, families, facility staff, and the Hospice Interdisciplinary Group (IDG) to develop spiritually and culturally sensitive care plans.
  • Provide and document spiritual care and counseling interventions, including sacraments and rituals from the patient's spiritual/religious community.
  • Document the provision of end-of-life information and education to facility staff, patients, and families in compliance with CMS Conditions of Participation.
  • Identify and refer patients of other faith perspectives to appropriate clergy or religious leaders in the community.
  • Provide spiritual support to hospice and partnering facility staff through counseling, educational in-services, and memorial services.
  • Participate in identifying clinical or operational performance improvement opportunities.
  • Maintain current knowledge of applicable spiritual care counselor practices.
  • Manage time, people, and technology professionally.
  • Participate in the orientation and training of new employees and students in all disciplines.
  • Collaborate with IDG and other TL departments to achieve patient care goals.
  • Maintain patient confidentiality at all times.
  • Perform other duties as assigned by supervisor.
