About the position

The Substance Addiction Counselor is a key team member responsible for providing comprehensive addiction counseling services to patients at Family Health Centers of Southwest Florida. This role involves conducting clinical evaluations, developing treatment plans, and delivering counseling and case management services under clinical supervision. The counselor will also collaborate with medical providers to integrate substance use counseling into patient care and ensure compliance with relevant regulations and standards.

Responsibilities

  • Conduct clinical evaluations and assessments for addiction counseling services.
  • Develop and implement treatment plans tailored to individual patient needs.
  • Provide addiction counseling and case management services to patients.
  • Consult with medical providers to integrate substance use counseling into medical care.
  • Maintain accurate documentation of client treatment progress and comply with agency standards.
  • Process referrals for substance use health needs and document outcomes.
  • Attend community functions and provide outreach services as required.
  • Collaborate with team members to develop client-centered treatment plans.
  • Act as a liaison between patients and community agencies for healthcare coordination.

Requirements

  • Bachelor's Degree in a Social Work/Human Services field is preferred.
  • State of Florida Certified Addictions Counselor (CAC) certification to be obtained within six months.
  • Minimum one year experience as a Substance Use/Addictions Counselor.
  • Experience working with adults with co-occurring disorders.
  • Strong interpersonal skills to establish counseling relationships with clients.

Nice-to-haves

  • Bilingual skills in English/Spanish/Creole preferred.
