Overview: We are currently in search of a proactive and detail-oriented Care Coordinator. Your primary role will involve staffing and scheduling caregivers to assigned patients and ensuring that all tasks are accurately documented in our software-based system. The ideal candidate should understand the importance of task completion, possess excellent organizational skills, and provide consistent support to our patients and/or caregivers. Responsibilities: Welcomes visitors by greeting them, in person or on the telephone; answering or referring inquiries. Route incoming calls to proper departments. Direct visitors by maintaining employee and department directories; giving instructions. Maintains reception area by complying with procedures, rules, and regulations. Maintain continuity among work teams by documenting and communicating actions, irregularities, and continuing needs. Contributes to team effort by accomplishing related results as needed.
