About the position

The Special Education Transition Facilitator at Oklahoma City Public Schools is responsible for supporting students with disabilities in developing the skills necessary for competitive and inclusive employment. This role focuses on enhancing students' self-determination skills, tracking their progress, and collaborating with families and school staff to ensure positive outcomes in both school and post-school environments.

Responsibilities

  • Provides instruction using district-approved curriculum related to self-determination.
  • Uses assessment data to identify students' strengths and areas needing instruction.
  • Tracks students' progress in developing self-determination skills over time.
  • Reports on student, classroom, school, and district self-determination status and outcomes.
  • Provides opportunities for students to develop and practice self-determination skills.
  • Individualizes instruction based on student needs, including those with complex communication needs.
  • Involves families to integrate their unique preferences, beliefs, and values.
  • Collaborates with administrators and school staff to minimize disruption to the learning environment.
  • Problem-solves obstacles to student engagement.
  • Assists in monitoring local plan implementation, legal compliance, and data collection.
  • Establishes and maintains professional communication with parents, students, and district personnel.
  • Attends department meetings and serves on staff committees as required.
  • Pursues professional development activities aligned with district goals and initiatives.
  • Complies with policies established by federal and state law, State Board of Education rule, and district board policy.
  • Performs other duties as assigned.

Requirements

  • Associates in Arts Degree (AA) is required; Bachelor's Degree is preferred.
  • Experience in the education setting is preferred.
  • Possession of a valid operator's driver's license and acceptable driving record.
