About the position

The Community Support Specialist at Lutheran Family Services is responsible for providing and coordinating rehabilitative and support services to adults with primary mental health diagnoses. This role involves developing treatment plans, facilitating client goals, and ensuring comprehensive care through collaboration with various service providers. The position emphasizes crisis management, client advocacy, and maintaining a supportive environment for individuals facing mental health challenges.

Responsibilities

  • Develop integrated treatment team plans with clients, updating every 3 months or as needed.
  • Facilitate implementation of clients' goals related to community living, daily living skills, and vocational/educational skills.
  • Discuss discharge goals with clients and develop discharge plans.
  • Conduct required assessments and arrange transportation for clients to appointments.
  • Develop crisis response and safety plans for each client.
  • Assess client safety and respond to crisis and safety concerns as needed.
  • Coordinate treatment activities with services from other agencies and treatment providers.
  • Advocate for clients and partner with other service providers.
  • Maintain a caseload while meeting monthly program billing requirements.
  • Attend required meetings, training, and supervision sessions.
  • Document all services and communication per program guidelines.
  • Participate in Performance & Quality Improvement (PQI) activities as assigned.
  • Abide by HIPAA regulations and ensure confidentiality of clients.

Requirements

  • Bachelor's degree in Psychology, Sociology, or related Human Service field preferred.
  • In lieu of a bachelor's degree, two years of relevant experience or coursework in a Human Services field is acceptable.
  • Case management experience and crisis management experience preferred.
  • Valid driver's license and reliable vehicle for daily client transportation.
  • Excellent interpersonal, communication, and conflict resolution skills.
  • Strong time management and problem-solving skills.
  • Ability to act with integrity, professionalism, and confidentiality.

Nice-to-haves

  • Basic understanding of physical health issues and their impact on mental health/functioning.
  • Demonstrated understanding of and ability to work with people of diverse backgrounds.
  • Knowledge of community organizations related to human services.
