About the position

The Specialized Mentor of Community Supports plays a crucial role in overseeing individuals participating in a specialized skill-building program. This position focuses on developing daily activity plans, facilitating community integration, and providing support to individuals with developmental disabilities. The mentor is responsible for fostering positive relationships, ensuring safety, and promoting independence among the individuals served.

Responsibilities

  • Develop daily activity planning with individuals served and area supervisor.
  • Transport individuals to and from community sites and coordinate learning to access transportation independently.
  • Facilitate development of positive relationships between individuals and employers.
  • Maintain and submit accurate and timely records and reports, including incidents and daily progress.
  • Assist individuals in becoming part of the community through valued roles and relationships.
  • Respect cultural differences and provide services that fit with individuals' preferences.
  • Link individuals to community resources through mentoring and volunteer work experiences.
  • Build trust and productive relationships with individuals served, co-workers, and others through clear communication.
  • Use person-centered practices to assist individuals in making choices and planning goals.
  • Monitor individuals' physical and emotional health and communicate observations to guide services.
  • Identify risks and behaviors that can lead to a crisis and use effective strategies to prevent or intervene in crises.
  • Facilitate individuals learning their own behaviors that can lead to a crisis and strategies to manage those independently.
  • Help individuals manage risk and recognize how to stay safe on their own.
  • Maintain individuals' confidentiality and respect their rights.
  • Provide advocacy and empower individuals to advocate for themselves.
  • Practice wellness initiatives with individuals.
  • Help individuals manage day-to-day tasks necessary for an independent community-based lifestyle.
  • Establish relationships with community resource agencies and employers.
  • Facilitate and encourage strong peer-group support.

Requirements

  • High school diploma and at least one year of experience working with individuals with developmental disabilities required.
  • Bachelor's degree preferred or an equivalent combination of education and experience.
  • Valid Driver's License with three years of driving experience is required.

Nice-to-haves

  • Experience in community resource linkage and advocacy for individuals with disabilities.
