About the position

The Speech Language Pathologist at HarmonyCares is responsible for providing high-quality speech therapy services to patients in their homes. This role focuses on assessing patient needs, developing personalized care plans, and delivering therapy to improve communication and swallowing abilities. The position emphasizes collaboration with clinical supervisors and physicians to ensure comprehensive patient care.

Responsibilities

• Assess and evaluate patient needs/problems, identifying mutually agreed upon goals for the patient care plan.
• Provide speech therapy services including assessment, evaluation, procedures, teaching, and training activities as outlined in the patient care plan.
• Report patient status to Clinical Supervisor and referring Physician.
• Document daily changes and updates to the patient care plans, including discharge planning.
• Attend staff meetings, team conferences, and educational in-services per agency requirements.
• Perform all OASIS time point assessments per Medicare Criteria and submit recertification paperwork per policy and procedure.

Requirements

• A current unencumbered State Professional License.
• Current driver's license and proof of insurance.
• One-year experience as a home care, professional Speech Language Pathologist, and competence in performing comprehensive assessment and OASIS-C.
• The ability to make sound professional clinical judgment.
• The ability to assess and document patient needs and formulate individualized patient care plans to meet those needs.
• Proficiency in clinical skills.
• Proficiency in Microsoft Office.
