About the position

The Speech Language Pathologist (SLP) at Angels of Care Pediatric Home Health is responsible for implementing treatment programs to assist pediatric patients with cognitive, speech, language, feeding/swallowing, and social/emotional disabilities and delays. This role involves planning and administering speech therapy services in both home and community settings, ensuring high-quality care tailored to the needs of each patient and their families.

Responsibilities

• Perform evaluations and interpret assessment results to develop appropriate goals for each child.
• Create and implement speech therapy treatment plans in conjunction with the physician.
• Assist pediatric patients in developing or regaining cognitive, speech, language, feeding/swallowing, and social/emotional functioning.
• Implement skilled interventions to maximize the potential of each individual child.
• Educate, coach, and empower caregivers and families to carry over therapeutic activities into the child's daily routine.
• Coordinate with referral partners to provide services for children according to physician orders.
• Design and instruct patients, families, and caregivers in home exercise programs to ensure continuation of therapeutic plans after discharge.
• Document patient care services and care coordination in an electronic medical record system.
• Maintain patient confidentiality by keeping information secure.

Requirements

• ASHA certification
• State License
• Current CPR certification
• A minimum of 1 year of experience preferred
