About the position

The Speech-Language Pathologist (SLP) position involves performing evaluations, developing care plans, and providing therapeutic interventions to restore and maintain patients' speech, language, motor, and swallowing abilities. The role requires flexibility in scheduling and the ability to work in various patient care settings, while adhering to departmental policies and procedures. The SLP will also coordinate with Home Health Aides and participate in interdisciplinary case management to improve patient outcomes.

Responsibilities

  • Perform evaluations and determine SLP diagnoses.
  • Develop a plan of care and perform speech therapy procedures.
  • Conduct timely reassessments, including OASIS assessments.
  • Update and revise the plan of care in consultation with the physician and interdisciplinary team.
  • Provide therapeutic treatments and evaluate equipment needs as applicable.
  • Prepare clinical and progress notes that support skilled intervention and evaluate outcomes of care.
  • Plan for patient discharge and build collaborative relationships with a multi-disciplinary team.
  • Participate in multi-disciplinary team meetings, in-services, and case conferences.
  • Coordinate with Home Health Aides and complete required supervisory visit documentation.
  • Participate in training and support of team members, volunteers, and students.

Requirements

  • Masters Degree in Speech Language Pathology (Required)
  • 1 year of experience in Speech Language Pathology (Preferred)
  • Licensure as a Speech Language Pathologist (SLP) from ASHA upon hire (Required)
  • CPR/BLS Certification from AHA/ARC within 30 days (Required)
  • Licensure as a Speech Language Pathologist (SLP) from Virginia Department of Health Professions (VDHP) upon hire (Required)
  • Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) (Required)
