About the position

The Speech Therapist (PRN) at Activa Home Health Services LLC is responsible for evaluating and treating individuals with speech, language, voice, and fluency impairments, as well as cognitive impairments. This role involves conducting assessments, formulating treatment plans, educating patients and families, and maintaining detailed client records. The therapist will also participate in meetings regarding patient progress and contribute to individualized educational planning.

Responsibilities

• Perform hearing and/or speech and language examinations to assess the patient's degree of impairment.
• Evaluate hearing and/or speech and language test results.
• Assess barium swallow results and/or other test results and overall medical background information.
• Formulate treatment plans for speech, language, fluency, voice, or swallowing impairments.
• Educate patients and family members about communication techniques or strategies to cope with impairments.
• Write reports and maintain client information for evaluation, treatment progress, and discharge purposes.
• Perform administrative responsibilities, coordinate paperwork, write lesson plans, and schedule case management activities.
• Develop individual and/or group activities and/or programs to manage behavior, speech, language, or swallowing problems.
• Participate in and write reports for meetings regarding patients' progress.
• Participate in planning and implementing individualized educational planning meetings, in-service meetings, or intervention assistance team meetings.

Requirements

• Master's degree in Speech-Language Pathology or related field.
• Appropriate certifications and/or licensures.
• 3-5 years of related experience.
• Excellent verbal and written communication skills.
