About the position

The Sponsored Residential Host Family role involves providing a supportive and family-like home environment for individuals with intellectual and developmental disabilities. The position focuses on fostering independence, community integration, and personal growth through tailored care and support, ensuring the individual's safety and well-being while promoting skill-building and access to community opportunities.

Responsibilities

  • Provide a safe, clean, and pleasant living environment for the individual.
  • Ensure compliance with state, local, federal, and contractual requirements.
  • Care for the individual's hygiene needs and provide personal hygiene items.
  • Assist the individual in selecting and purchasing culturally appropriate clothing.
  • Ensure the individual receives three nutritious meals per day and snacks, including training in meal preparation and cleanup.
  • Provide adult supervision at all times when the individual is in the home.
  • Transport the individual to health professionals and assist in following doctor's orders.
  • Participate in the development of the individual's service plan and provide necessary training.
  • Maintain required records such as documentation of fire drills and monthly reports.
  • Work cooperatively with the individual's family or advocate and arrange for visits as needed.
  • Include the individual in family recreation and leisure activities regularly.
  • Ensure confidentiality of the individual's records and health information.
  • Permit access to Merakey staff for supervision and emergency purposes.
  • Share important decisions affecting the individual's welfare with Merakey staff.
  • Notify the agency of any changes in address or telephone number at least 30 days in advance.
  • Report any changes in Sponsored Residential arrangements before lodging other individuals in the home.
  • Notify the agency before taking the individual for extended trips and inform about substitute care arrangements.
  • Attend all provider family group meetings for training and support.
  • Complete required trainings.

Requirements

  • High school diploma or GED (minimum).
  • Experience in caregiving or related human services roles preferred.
  • Strong communication, problem-solving, and organizational skills.
  • Ability to provide a stable and supportive home environment.
  • Completion of required training and ability to comply with regulatory standards.
