About the position

The Senior Staff Pharmacist at Houston Methodist is responsible for providing comprehensive, patient-oriented pharmacy services, including proactive patient profile reviews, discharge counseling, and medication histories. This role involves collaboration with the healthcare team to ensure effective pharmaceutical care, mentoring pharmacy staff, and participating in departmental quality management activities. The position emphasizes a commitment to quality, safety, and innovative pharmaceutical care.

Responsibilities

  • Proactively review patient profiles and participate in clinical functions such as discharge counseling and medication histories.
  • Provide day-to-day comprehensive pharmacy services, including processing orders and therapy reviews.
  • Serve as a role model and preceptor for staff pharmacists, pharmacy students, and residents.
  • Collaborate with the patient care team to communicate pertinent patient care information.
  • Guide and mentor pharmacy technicians and new pharmacists to build their confidence and skills.
  • Develop educational materials and programs for pharmacy staff and hospital personnel.
  • Monitor workload fluctuations and adjust assignments for pharmacy technicians to ensure timely medication delivery.
  • Conduct proactive drug therapy reviews and provide drug information and patient counseling.
  • Document patient care activities and outcomes impacted by pharmaceutical care and clinical interventions.
  • Participate in department quality management activities, including medication use evaluations and adverse drug reactions reporting.

Requirements

  • Graduate of an education program approved by the credentialing body for required credentials.
  • Successful completion of an ASHP-accredited PGY1 residency training program or three years of relevant pharmacist experience.
  • One year of experience precepting pharmacy students and/or residents.
  • Texas Pharmacist license or eligibility within 90 days.
  • Pharmacist Preceptor Certification within 90 days.
  • Neonatal Resuscitation certification within 90 days for those responding to Code Blue events.
  • Intravenous certification (ACPE) within 90 days if applicable to primary work assignment.
  • Basic Life Support certification (AHA) within 90 days.
