About the position

The SS Director is responsible for establishing a course of action to assist residents in achieving their highest level of self-care, independence, and well-being. This role involves providing medically related social services to ensure the physical, mental, and psychosocial well-being of each resident is maintained. The SS Director evaluates social and family information, addresses emotional needs, and coordinates with various agencies to meet residents' needs effectively.

Responsibilities

• Establish course of action by exploring options and setting goals with residents and their families.
• Assist residents in achieving the highest practicable level of self-care, independence, and well-being.
• Provide medically related social services to maintain residents' physical, mental, and psychosocial well-being.
• Evaluate social and family information to assess social services needs and develop care plans.
• Provide emotional support and address emotional problems for residents and families.
• Assist residents with health care decisions and personal shopping.
• Track and inventory patient belongings, coordinating responses to reports of missing items.
• Assist in obtaining resources from community and social services agencies.
• Secure appropriate prosthetics and assistive devices for residents.
• Develop and maintain relationships with other departments and community agencies.
• Document resident social service status and assist in making outpatient appointments.
• Coordinate discharge planning with appropriate agencies and individuals.
• Provide in-service training to staff as requested.
• Counsel residents and family members.

Requirements

• Bachelor's Degree in Social Work or Human Services.
• 2 years of supervised social work experience in a health care setting working directly with individuals.
• Knowledge and experience with PCC preferred.

Nice-to-haves

• MSW preferred.
