About the position

The Remote Clinical Manager is responsible for overseeing clinical operations in care centers, ensuring quality patient care while adhering to fiscal responsibility and compliance with regulatory standards. This role involves coordinating with various stakeholders, managing patient care plans, and monitoring clinical performance to achieve key performance indicators.

Responsibilities

  • Oversees scheduling activities to ensure patient needs are met.
  • Reviews assignments with the Patient Services Coordinator and staff.
  • Oversees payor verification and pre-certification requirements.
  • Communicates with payors and responds to requests for information.
  • Reviews clinician documentation to ensure appropriateness of care and compliance with documentation requirements.
  • Provides feedback to clinicians regarding documentation and performance.
  • Assists clinicians in establishing patient goals and developing Plans of Care (POC).
  • Manages and delivers patient care, including utilization management and patient outcomes.
  • Ensures timely communication of changes in patient condition to physicians.
  • Evaluates and revises patient plans of care in collaboration with clinicians.
  • Leads patient care conferences to promote coordination of care.
  • May provide direct patient care in emergencies.
  • Participates in after-hours on-call process as needed.
  • Engages in quality assurance activities to improve patient care.
  • Responds to Additional Documentation Requests/Denials as directed.
  • Instructs and guides clinicians to enhance performance and service delivery.
  • Coordinates activities with other disciplines to ensure continuity of care.
  • Investigates and resolves patient/family/physician concerns.
  • Conducts training for new hires and mentors Clinical Managers.
  • Recommends employment decisions to the Director of Operations.
  • Performs other duties as assigned.

Requirements

  • Associate or Bachelor's degree in Nursing.
  • Current, unencumbered license to practice as a Registered Nurse in the assigned state(s).
  • One (1+) year of supervisory and/or management experience as a Registered Nurse in a healthcare environment.
  • One (1+) year of RN experience in a Home Health or Hospice environment.
  • Current CPR certification.
