About the position

The Stabilization Case Manager plays a crucial role in providing ongoing case management services to families transitioning from shelter to permanent housing. This position focuses on continuous barrier reduction to ensure long-term success for families in the Stabilization Program, which typically lasts up to three years. The Case Manager assists families in developing essential skills and resources to sustain their housing and navigate various challenges they may face during this transition.

Responsibilities

• Monitor intake, assessment, and data entry for all program clients using appropriate forms and assessment tools.
• Maintain regular communication with the Property Owner to ensure program compliance.
• Assist families with tenancy concerns, including budget planning and reporting maintenance issues to the property owner.
• Help families enroll in entitlement benefits such as SNAP, TAFDC, and SSI.
• Collaborate with community partners to enhance support for families.
• Conduct regularly scheduled meetings with families to discuss progress and concerns.
• Keep the supervisor informed of any concerns or potential problems.
• Attend relevant meetings and trainings as required.
• Perform other duties as assigned by the Shelter Manager or Director of Shelter Case Management.

Requirements

• Associate's or Bachelor's degree in Human Services or a related field.
• Minimum of two (2) years of experience in case management or a similar role.
• Demonstrated case management skills.
• Strong written and verbal communication skills.
• Excellent interpersonal skills.
• Empathy towards the needs of a diverse socio-economic population.
