About the position

Advocate Aurora Health is seeking a per diem / prn Float Physical Therapist in Hartford, Wisconsin. This role involves providing therapy services to patients, interpreting physician referrals, and establishing individualized treatment plans. The therapist will monitor patient progress, complete necessary documentation, and ensure a clean and organized treatment area. The position requires effective communication and the ability to manage a caseload while participating in professional development activities.

Responsibilities

  • Interprets physician referrals and completes evaluations utilizing appropriate methods.
  • Establishes a plan of care and implements individual treatment based on evaluation results and functional goals.
  • Monitors and evaluates outcomes and adjusts the treatment plan based on patient response.
  • Identifies factors affecting patient's physical and psychosocial comfort and attends to these needs.
  • Plans for and completes patient discharge, providing written home programs and referrals for community services.
  • Completes all required documentation following department policy, including patient progress notes and discharge summaries.
  • Applies insurance guidelines regarding authorization and number of visits.
  • Responsible for cleaning up after patient treatment and keeping the area neat and orderly.
  • Performs various manual techniques, patient positioning, transferring and transporting duties.
  • Demonstrates skill in the use of modalities, exercise equipment, and other devices available in the area worked.
  • Participates in performance improvement activities to improve quality of care.
  • Manages assigned caseload and informs site leadership of coverage needs.
  • Pursues professional growth through continuing education and department inservices.
  • Orients new staff to clinical and department policies as requested.

Requirements

  • Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless determined otherwise.
  • Physical Therapist license issued by the state of Wisconsin.
  • Bachelor's Degree in Physical Therapy.
  • No experience required.
  • Intermediate computer skills.
  • Excellent communication and interpersonal skills.
  • Demonstrated organization, decision-making, and problem-solving skills.
