About the position

The Staff Nurse (LPN) at Chapel Hill Community is responsible for coordinating care delivery interventions for approximately 25 residents, ensuring compliance with professional standards, physician's orders, and regulatory requirements. This part-time role involves managing resident treatments, administering medications, and participating in the assessment and planning of resident care, while also providing education and support to families.

Responsibilities

  • Manage resident treatments and administer prescribed medications and treatments according to approved nursing protocols.
  • Assess residents under the supervision of an RN and participate in care planning and evaluation.
  • Make resident rounds to ensure quality care delivery and adherence to restorative protocols.
  • Utilize infection control techniques to prevent the spread of infection.
  • Observe vital signs and hydration status, and document changes in condition.
  • Assist in receiving, transcribing, and implementing physician's orders.
  • Request and arrange for diagnostic and therapeutic services as ordered by the physician.
  • Order prescribed medications and requisition necessary supplies and equipment.
  • Document resident responses to treatments and medications, and report findings to supervisors and physicians.
  • Oversee the execution of various nursing procedures, including catheter care and tube feeding.
  • Monitor residents' dietary needs and weight changes, and report any concerns to the physician.
  • Complete medical forms, reports, and evaluations as necessary, ensuring accurate documentation of care provided.
  • Participate in care conferences and assist in the completion of care plans.
  • Educate residents on post-discharge needs and ensure proper discharge processes are followed.

Requirements

  • Valid LPN license conferred by the State Board of Nursing.
  • Successful completion of a state-approved pre-licensure nursing education program.
  • One year of experience in nursing and/or long-term care preferred.
  • CPR certification required.
  • Good knowledge of pharmacology and treatments, especially concerning the elderly and dementia.

  • Experience in weight loss management.
  • Demonstrated people skills, particularly in interfacing with patients' family members.
  • Working knowledge of different cultures and religions.
