About the position

The Staff RN - Disease Specific Program Coordinator at Legacy Health plays a crucial role in enhancing patient care for specific disease populations. This position is designed for a registered nurse who will work closely with both physician and administrative leadership to ensure optimal care delivery at a site-based level. The coordinator will collaborate with peers across the Legacy Health system to integrate best practices and promote a cohesive approach to patient care. As a clinical expert, the coordinator will serve as a resource for staff involved in disease-specific patient care, facilitating team coordination and the provision of evidence-based care. In this role, the coordinator will be responsible for developing and maintaining disease-specific protocols and processes, working in conjunction with Service Line leadership when applicable. The position also emphasizes the importance of patient and community education, ensuring that patients and their families are well-informed about their conditions and care options. The coordinator must prioritize their work effectively, managing time and projects with minimal supervision to meet the demands of the role. Responsibilities

  • Work with physician and administrative leadership to promote optimal patient care for a disease-specific patient population.
  • Collaborate with peers across Legacy Health to integrate best practices throughout the organization.
  • Serve as a clinical expert resource for staff providing disease-specific patient care.
  • Facilitate team coordination in the provision of evidence-based care.
  • Develop and maintain disease-specific protocols and processes to support care provision in conjunction with Service Line leadership.
  • Support patient and community education.
  • Prioritize work and effectively manage time and projects with minimal supervision.

Requirements

  • Academic degree in nursing (BSN or MSN/MN) required.
  • Two years of experience in the provision and coordination of care for the disease-specific patient population required; three years preferred.
  • Experience leading performance improvement efforts and disease-specific program management strongly preferred.
  • Demonstrated skills in patient assessment, multidisciplinary collaboration, problem solving, data analysis, leading change, teaching, and coaching.
  • Proficiency in Microsoft Office Suite and clinical information systems.
