About the position

The State Tested Nursing Assistant (STNA) position at Hospice of the Western Reserve is a full-time role focused on providing personal care and support to patients in hospice settings. The STNA will work under the supervision of a nurse, assisting with daily living activities, ensuring patient safety, and contributing to the overall quality of life for patients and their families. This role emphasizes compassion, teamwork, and adherence to care protocols, with a commitment to enhancing comfort and dignity in end-of-life care.

Responsibilities

  • Provide personal care and assist with Activities of Daily Living (ADLs) according to the Plan of Care (POC).
  • Ensure a safe environment for patients.
  • Safely ambulate and transfer patients as appropriate.
  • Assist with passive and active Range of Motion (ROM) exercises as directed by the nurse.
  • Prepare nutritional snacks for patients as appropriate.
  • Assist patients with pre-poured medications that are self-administered under nurse supervision (Home Care only).
  • Perform simple wound care, skin care, and bowel care protocols as directed by the nurse.
  • Recognize and report any changes in the patient's mental or physical status to the nurse.
  • Document activities and observations in the clinical record during visits.
  • Complete all required charting, timesheets, and mileage logs as per agency policy.
  • Attend and contribute to patient care conferences and team meetings.
  • Participate in on-call rotation as scheduled (Home Care only).
  • Assist the Team Leader with scheduling, supply organization, and other designated activities.
  • Demonstrate positive customer service responsiveness according to agency standards.
  • Be flexible in meeting patient and family needs, including variable shifts and weekends.
  • Participate in quality improvement activities and chart reviews.
  • Coordinate and communicate with volunteer staff.
  • Provide basic caregiving education to family members, such as transfer techniques and changing linens.
  • Maintain Transmission Based Precautions and complete annual competency requirements.

Requirements

  • Must be a State Tested Nursing Assistant in good standing.
  • Proficient in reading and writing English.
  • Valid driver's license and active auto insurance policy in Ohio.
