About the position

The Street Health Outreach RN Care Manager at Tiburcio Vasquez Health Center plays a crucial role in providing nursing care and support to individuals experiencing homelessness in Central Alameda County. This position involves conducting health assessments, administering medical care, and collaborating with a multi-disciplinary team to improve the health and well-being of clients. The RN Care Manager will also supervise community health workers and engage with various stakeholders to enhance service delivery.

Responsibilities

  • Provide nursing services including administration of medications and injections as directed by providers.
  • Conduct health assessments and provide brief interventions and triage for patients.
  • Administer medical care in unhoused encampments and TVHC's provided van.
  • Prioritize individuals with high medical and behavioral health needs for intensive visits.
  • Assess learning needs of patients and develop appropriate patient education.
  • Act as a resource and contact person for patients, responding to their immediate survival needs.
  • Provide medical oversight of Community Health Worker and Behavioral Health Case Manager.
  • Link and refer patients to community services as needed.
  • Represent TVHC at county and community meetings and engage with community stakeholders.
  • Participate in multidisciplinary staff meetings for program development and evaluation.
  • Maintain confidentiality and complete required documentation in a timely manner.
  • Build relationships with potential patients through consistent outreach and engagement.

Requirements

  • Current, valid California RN license.
  • Current, valid CPR certification.
  • Valid California driver's license and insurance.
  • Preferably two years of nursing experience serving patients with complex needs, ideally in unsheltered populations.
  • Bilingual in English, Spanish, or other languages is preferred.
  • Ability to work independently and collaboratively as a team member.
  • Demonstrated experience with quality improvement processes.
  • Culturally sensitive with effectiveness in working with ethnically diverse populations.
  • Ability to effectively intervene in crisis situations with de-escalation techniques.
  • Excellent communication and documentation skills.
