About the position

The Substance Use Disorder (SUD) Case Manager is responsible for providing comprehensive case management services to adults facing behavioral health challenges and substance use issues in Washington County. This role involves assessing client needs, developing tailored treatment plans, and connecting clients with necessary resources to support their recovery and stability in various aspects of life, including housing, employment, and education.

Responsibilities

• Conduct assessments to evaluate the strengths and needs of individuals.
• Analyze situations and problems to determine required services.
• Address basic client needs alongside therapeutic interventions.
• Formulate, implement, evaluate, and revise goal-oriented treatment plans.
• Develop crisis and safety plans for clients.
• Create and facilitate a support team for clients to monitor progress.
• Research resources and services to assist clients and their families.
• Safely transport clients as needed.

Requirements

• Bachelor's Degree in Social Work, Psychology, Criminal Justice, or a related field.
• Minimum of two years of experience working with individuals with mental health and addiction issues.
