About the position

The Substance Use Disorder Counselor/Case Manager II at Family Health Centers of San Diego (FHCSD) plays a crucial role in providing comprehensive substance abuse counseling and case management services to individuals in need. This position focuses on delivering therapeutic interventions, conducting assessments, and developing individualized care plans for clients, particularly those from underserved communities. The counselor will also provide leadership and training to staff, ensuring effective treatment and support for patients dealing with substance use disorders.

Responsibilities

  • Conduct initial and ongoing assessments of clients' health and support service needs.
  • Create formalized case plans and goals with clients requiring intensive services.
  • Provide substance abuse screening and assessments.
  • Participate in the development of individualized care plans for patients.
  • Assess patients' needs for enrollment in medical, self-help programs, outpatient services, and residential facilities, making referrals as necessary.
  • Update care plans as clients' needs change.
  • Provide therapeutic interventions and intensive individual support to clients.
  • Offer substance abuse education and referral information to patients, families, and staff.
  • Complete all required documentation accurately and in a timely manner.
  • Develop and maintain community resources information and contacts.
  • Act as a liaison and consult with community agencies.
  • Participate in the design and implementation of mental health and substance abuse service delivery with a multi-disciplinary team.
  • Provide direct substance abuse counseling services, including crisis intervention and individual, group, and family counseling.

Requirements

  • Bachelor's degree in Social Science, Public Health, Healthcare Administration, or a closely related field required.
  • Master's degree in a relevant field preferred.
  • High School diploma or GED equivalent required.
  • Registered Intern status or certification from a California State Substance Use Disorders (SUD) Services certifying agency required.
  • 1 year of experience in providing counseling required; 2 years preferred with specific patient demographics.
  • 3 years of work experience providing human services to high-risk, medically underserved populations required.

Nice-to-haves

  • Experience with patients diagnosed with serious mental illness or HIV preferred.
