About the position

The Substance Use Specialist in Assertive Community Treatment (ACT) plays a crucial role in integrating dual-recovery treatment and harm reduction strategies within a multi-disciplinary team. This position focuses on supporting individuals facing challenges such as homelessness, trauma, and substance use disorders, helping them achieve clinical stability and improved quality of life through evidence-based practices and community resources.

Responsibilities

  • Develop rapport with clients to engage them in improving their health and wellness.
  • Administer standardized health and psychosocial risk screenings according to program protocols and timeframes.
  • Administer standardized substance abuse assessment screening tools including SBIRT, AUDIT, and DAST, integrating dual-recovery treatment and harm reduction strategies.
  • Utilize health screenings to identify interventions and develop a comprehensive care plan.
  • Collaborate with care team members to identify needs and develop a plan for optimal health outcomes.
  • Develop WRAP and Relapse Prevention Plans to help clients understand their patterns and triggers for use.
  • Utilize Contingency Management to identify behavioral objectives and issue rewards as objectives are achieved.
  • Implement tasks outlined on the care plan and ensure follow-up and continuity of care between client interactions.
  • Regularly review and update the care plan to correspond with services being provided.
  • Document all interventions and attempted contacts in the EHR in accordance with program standards.
  • Assess domiciled clients' living conditions by conducting home visits.
  • Work with family members and other collaterals of the client's choice to facilitate planning and delivery of care.
  • Provide comprehensive transitional care following hospitalization events in accordance with ACMH Critical Time Intervention (CTI) Protocols.
  • Review new information and complex cases with PCP and multidisciplinary team and incorporate recommendations into the care plan.
  • Facilitate care delivery by scheduling appointments, obtaining necessary information, and arranging transportation.
  • Identify local resources and facilitate referrals to needed services such as self-help groups and detox/rehab services.
  • Utilize evidenced-based practices, such as motivational interviewing and cognitive behavioral techniques, to empower clients to grow and attain goals.
  • Embrace the team model by collaborating with team members and providing support as needed.
  • Support client goals and serve as an advocate on the client's behalf.
  • Administer Client Service Dollar funds and submit required documentation.
  • Regularly participate in team meetings and weekly clinical conferences.
  • Attend in-service training as requested.

Requirements

  • Master's degree in social services or a related field.
  • One year of experience working with adults with substance use disorders or co-occurring substance use and mental health disorders.
  • Demonstrated competency in integrated mental health and substance abuse assessment and treatment.
  • Strong written and verbal communication skills, including competency in clinical documentation.
  • CASAC or licensed clinician required.

Nice-to-haves

  • Experience in documentation review.
  • Familiarity with evidence-based practices in substance use treatment.
