About the position
The Supervisor of Case Management at St. Mary's General Hospital is responsible for overseeing the Case Managers, Coordinators, and Discharge Planners. This role ensures quality and resource management for all patients admitted to the facility, advocating for cost containment and promoting a seamless delivery of care across the healthcare continuum.
Responsibilities
- Supervise Case Managers, Coordinators, and Discharge Planners.
- Oversee quality and resource management for admitted patients.
- Advocate for cost containment in patient care.
- Integrate healthcare providers to ensure seamless delivery of care.
Requirements
- Valid NJ RN licensure required.
- CCM obtained within 6 months.
- Minimum of 2 years' experience in Case Management and discharge planning in an acute care environment.
- Experience and knowledge in basic to intermediate computer skills.
Nice-to-haves
- Minimum 5 years of acute care experience preferred.
- Current BCLS (AHA) certificate preferred.
- Knowledge of Milliman Criteria and InterQual Criteria preferred.
