About the position

The Supervisor of Case Management Services will lead a multi-disciplinary team within a Scattered Site Apartment Program aimed at supporting mentally ill adults. This role involves overseeing the delivery of rehabilitative services, ensuring compliance with documentation and billing requirements, and fostering the development of independent living skills among residents.

Responsibilities

  • Oversee the daily provision of rehabilitative services to residents.
  • Prepare weekly reports for the Program Director.
  • Maintain professional relationships with service providers and develop new resources for client training and recreation.
  • Assist staff in developing Admission Notes, Service Plans, and handling crises.
  • Participate in internal and external Case Conferences and provide in-service training.
  • Work with senior staff to ensure clinically appropriate Intake and Discharge processes.
  • Sign off on Admission Notes and monitor new cases with the Medicaid Specialist.
  • Provide regular supervision of all Case Managers and conduct weekly group meetings.
  • Develop a monthly schedule to assess service delivery quality through direct observation.
  • Ensure payment schedules with residents and maintain Benefits/Entitlements.
  • Be available for emergency contact on a 24-hour basis.
  • Supervise Case Managers in Quality Assurance monitoring and maintain orderly case records.
  • Plan and monitor apartment visitation schedules for Case Managers.
  • Ensure resident involvement in Service Plans and program development.
  • Conduct monthly Chart Reviews and participate in Agency-wide Q.A. Committees.
  • Facilitate special activities that encourage community integration.
  • Participate in new Staff Orientation and ensure staff develop crisis intervention and case management skills.

Requirements

  • Masters degree in Social Work or related field preferred.
  • A minimum of five years experience in a mental health facility or community residence.
  • A minimum of two years in a supervisory position required.
  • Excellent organizational, administrative, and supervisory skills.
  • Knowledge of Psych Rehab principles is a must.
  • Excellent interpersonal and communication skills.
