About the position
The Supervisor of Social Work - Case Management at Providence is responsible for overseeing the daily operations and management of assigned Social Workers. This role ensures excellence in customer service and the timely provision of coordinated patient care, including care coordination, discharge planning, and psycho-social assessments. The Supervisor develops operational plans, manages staffing resources, and implements quality improvement activities under the direction of the Case Management Manager.
Responsibilities
- Directly supervise assigned Social Workers and coordinate their daily activities.
- Develop and implement operational plans for staffing, policies, and quality improvement.
- Ensure timely provision of coordinated patient care, including social work functions.
- Maintain expertise in social service processes and align services with departmental goals.
- Foster a culture of patient-focused, whole-person care within the team.
Requirements
- Master's Degree in Social Work.
- Washington Advanced Social Worker License or Washington Clinical Independent Social Worker License upon hire.
- 3 years of recent experience in acute inpatient care with diverse patient populations.
- Previous Social Work experience.
