About the position
The Supervisor of Social Work - Case Management at Providence St. Joseph Health is responsible for overseeing the daily operations and management of assigned Social Workers. This role involves ensuring excellence in customer service, developing operational plans, and coordinating patient care through social work functions such as care coordination and discharge planning. The Supervisor plays a crucial role in maintaining high standards of care and aligning services with departmental goals.
Responsibilities
- Directly supervise assigned Social Workers and manage their daily activities.
- Develop and implement operational plans including staffing resources and policies.
- Ensure timely provision of coordinated patient care through social work functions.
- Conduct psycho-social assessments and interventions in accordance with social work standards of care.
- Maintain expertise in social service processes and ensure alignment with departmental vision and goals.
Requirements
- Master's Degree in Social Work.
- Washington Advanced Social Worker License or Washington Clinical Independent Social Worker License upon hire.
- 3 years of recent experience with diverse patient populations in acute inpatient care areas.
- Previous Social Work experience.
