About the position

The Clinical Pharmacist plays a crucial role in delivering optimal medication management and ensuring safe medication use within the correctional health system. This position is responsible for interpreting and evaluating medication orders, providing pharmacy services to healthcare staff, and integrating clinical pharmacy knowledge into practice. The Clinical Pharmacist will also engage in multi-disciplinary rounds, direct patient care, and educational activities, while serving as a preceptor for pharmacy interns and residents.

Responsibilities

  • Deliver optimal medication management across all dimensions of care.
  • Ensure safe medication use and compliance with legal, accreditation, and certification requirements.
  • Provide pharmacy services to patients, nurses, physicians, and other medical staff.
  • Interpret and evaluate the validity and safety of medication orders and prescriptions.
  • Perform core clinical activities including order entry, pharmacokinetics, IV to PO conversions, and therapeutic interchange.
  • Participate in multi-disciplinary rounds and clinical initiatives.
  • Provide education and training to pharmacy technicians, pharmacists, and other healthcare practitioners.
  • Serve as a preceptor to pharmacy interns, technicians, and ASHP-accredited pharmacy residents.

Requirements

  • PharmD degree from an accredited institution.
  • Registered with the Texas State Board of Pharmacy; must obtain Texas license within 90 days if licensed by reciprocity.
  • ASHP accredited PGY1 residency required.
  • Board of Pharmacy Specialties Certification strongly preferred.
  • ACPE IV Sterile Certification required.
  • Completion of a residency program or a minimum of 2 years of clinical practice experience.
  • Proficient in writing and report generation using MS Word.
