The Support Coordinator for the Developmental Disabilities Division (DDD) plays a crucial role in assisting individuals with developmental disabilities in accessing and navigating community services and resources. This position involves assessing client needs, developing individualized service plans, coordinating services, and ensuring clients receive appropriate care to enhance their quality of life. The Support Coordinator collaborates with clients, families, service providers, and community organizations to effectively meet all client needs.

  • Conduct comprehensive assessments to understand the needs, goals, and preferences of individuals with developmental disabilities.
  • Develop and update individualized service plans (ISPs) to ensure appropriate services are in place.
  • Collaborate with service providers to ensure timely delivery of services, including healthcare, vocational, residential, and educational support.
  • Regularly monitor clients' progress towards goals and the quality of services they are receiving.
  • Advocate for clients to ensure they have access to all services and resources available through the DDD program and other community-based resources.
  • Maintain accurate and up-to-date records of client assessments, service plans, progress notes, and other required documentation.
  • Work closely with families, caregivers, healthcare professionals, and other stakeholders to coordinate care.
  • Provide information and education to clients and their families about available services, rights, and community resources.
  • Assist clients and families in addressing urgent needs and crises, providing timely interventions and coordination of necessary services.
  • Ensure all services and actions comply with federal, state, and local laws, including DDD program policies and procedures.
  • Bachelor's degree in Social Work, Psychology, Special Education, or related field (required).
  • Minimum of 2 years of experience working with individuals with developmental disabilities, including experience in case management or service coordination.
  • Strong knowledge of developmental disabilities, available services, and community resources.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work independently, manage multiple tasks, and maintain accurate documentation.
  • Ability to advocate for clients and work collaboratively with families and service providers.
  • Knowledge of relevant state and federal regulations governing the DDD program.
  • CPR/First Aid certification (or willingness to obtain).
  • Certification as a Support Coordinator or case manager in the developmental disabilities field.
  • Experience working with Medicaid or state-funded programs.
  • Bilingual (Spanish, ASL, etc.) a plus.
  • Competitive hourly pay of $40.00 per hour.
