About the position

The Supportive Care Operations Coordinator at Southcoast Health is responsible for overseeing the daily operations of Supportive Care Services, ensuring efficient patient scheduling, managing administrative functions, and acting as a liaison among patients, referral sources, and clinical teams. This role is integral to maintaining high-quality care and operational efficiency within the hospice program.

Responsibilities

• Oversee daily operations of Supportive Care Services under the supervision of the Director.
• Manage patient scheduling, including processing referrals and case assignments.
• Prioritize visits and clinician assignments, coordinating admissions as needed.
• Handle administrative functions such as managing medical supplies and medications.
• Clinically triage incoming patient calls and physician orders.
• Act as a liaison between patients, referral sources, intake staff, and the clinical team.

Requirements

• Current LPN licensure in the Commonwealth of MA is required.
• CPR and Basic Life Support (BLS) certifications are required.
• Five (5) years of nursing experience, preferably in Homecare or Hospice required.
• Supervisory experience, preferably in a Hospice setting, is desired.
• Must possess and demonstrate highly effective interpersonal, customer service, and communication skills.
• Proficient computer skills, including experience with various software, database, and electronic records programs is required.
