About the position

The Supportive Housing Case Worker at New Reach plays a crucial role in providing supportive housing services to individuals and families at risk of homelessness or currently experiencing it. This position focuses on collaboration with community stakeholders to deliver essential services aimed at stabilizing housing and promoting independence for low-income clients affected by mental health and substance use issues. The case worker serves as the first point of contact, offering advocacy, housing counseling, and community linkages to ensure clients receive the necessary support during critical transitions.

Responsibilities

  • Accept referrals from local and regional sources to engage new clients in services.
  • Complete program intakes and conduct comprehensive, strengths-based assessments of needs.
  • Co-create person-centered goal plans with clients and involved parties.
  • Provide home and community-based case management services, including crisis intervention and life skills training.
  • Assist clients with affordable housing searches and tenancy skill building.
  • Identify unaddressed needs and encourage access to integrated support services.
  • Engage with property managers to advocate for housing rights and mediate concerns.
  • Maintain collaborative relationships with program partners and community providers.
  • Utilize HMIS database and other tools to document assessments and client outcomes.
  • Participate in meetings and develop a strong community profile with relevant service providers.

Requirements

  • Bachelor's degree in Human Services or related field, or Associate's degree with 2 years of experience, or Certified Recovery Specialist with 2 years of experience.
  • Valid driver's license and access to reliable transportation.
  • Excellent interpersonal, oral, and written communication skills.
  • Ability to provide intensive services to households experiencing homelessness.
  • Strong organizational skills to manage multiple projects.
  • Proficiency in Microsoft Outlook, Word, Excel, and PowerPoint.

Nice-to-haves

  • Experience with housing law and provision of services to the identified population.
  • Experience working with individuals affected by mental health or substance use conditions.
  • Bilingual in Spanish, both written and oral.
