About the position

The Supportive Housing Program Case Manager at the Greensboro Housing Authority (GHA) is responsible for providing case management and support services to individuals and families in the Supportive Housing and Shelter Plus Care Programs. This role focuses on assisting a special needs population, including the chronically homeless, severely mentally ill, chronic substance abusers, veterans, persons with HIV/AIDS, and victims of domestic violence. The case manager will help participants establish and maintain housing, define goals, and identify supportive services to enhance their success.

Responsibilities

  • Provide case management and support services in one-on-one and group sessions.
  • Assist individuals and families in the Supportive Housing and Shelter Plus Care Programs.
  • Monitor program participants in establishing or maintaining housing.
  • Define goals and identify supportive services for participants.
  • Maintain general files for records and correspondence.
  • Keep records in the North Carolina Homeless Management Information System (NCHMIS) database.
  • Assist in budget preparation and perform general office duties.
  • Exercise tact and courtesy in dealing with outside agencies and the general public.

Requirements

  • Bachelor's degree in social work or a related field.
  • Minimum of one year of experience as a social worker, preferably with the homeless and at-risk populations.
  • Possession of a valid driver's license issued by the state of North Carolina with a safe driving record.
