About the position

The Supportive Services for Veteran Families (SSVF) Case Manager at Midwest Shelter for Homeless Veterans is a full-time role focused on providing case management services to homeless veterans and their families. This position is critical in helping clients achieve housing stability and access necessary benefits, while working within a compassionate and structured environment. The Case Manager will collaborate closely with the SSVF Program Manager and team to ensure adherence to program standards and to empower clients towards self-sufficiency.

Responsibilities

  • Conduct screening and assessment for literally homeless veterans and their families.
  • Perform regular outreach to emergency shelters, Veterans Assistance Commissions, VSOs, and VA Medical Centers to identify and engage homeless veterans.
  • Provide assessment, outreach, case planning, housing advocacy, eligibility determination, tenant education, and case management/housing stabilization services to families in the SSVF Program.
  • Assist clients in obtaining mainstream and veteran-specific benefits.
  • Accurately enter client data into an established HMIS system.
  • Cultivate relationships with current and potential landlords to maximize housing units available to families.
  • Provide aftercare services to families who have stabilized in housing and exited the program.
  • Represent MSHV to local, state, and national organizations as assigned.
  • Network with the community to meet client needs and advocate for clients.
  • Manage multiple tasks effectively and respond to emergencies with appropriate crisis intervention techniques.
  • Prepare and maintain accurate confidential case records and fiscal documentation as required.

Requirements

  • Master's Degree in Social Work or related Human Services field (equivalent work experience may be considered).
  • At least 1 year of experience working in the field.
  • Skilled in Microsoft Office (Excel, Word, Outlook, PowerPoint).
  • Exceptional communication and problem-solving skills.
  • Ability to instill confidence and inspire commitment to organizational goals.
  • Strong organizational and follow-up skills.

Nice-to-haves

  • Veteran status is a plus.
  • Previous experience working with homeless individuals and families, particularly chronic homelessness.
  • At least one year of experience in a program serving veterans is preferred.
  • Experience with Housing First/Harm Reduction approach.
