About the position

The Support Coordinator / Case Manager at Care Lync is responsible for providing essential case management services to adults with Intellectual, Developmental, and Physical Disabilities, as well as Autism Spectrum Disorder. This role involves developing individualized care management plans, coordinating services, and monitoring clinical outcomes to ensure clients receive comprehensive and effective support. The position requires significant interaction with clients to help them navigate and secure necessary services for independent living.

Responsibilities

  • Complete assessments to assist in identifying each participant's needs and desires for service delivery.
  • Develop a Person-Centered Service Plan for each participant and modify it at least annually or when there is a significant medical or social change.
  • Ensure the participant's choice of providers by providing information for informed decision-making.
  • Ensure the Behavioral Support Plan and Crisis Plan is consistent with the Individual Support Plan (ISP).
  • Report all incidents of participant abuse, neglect, or exploitation immediately to supervisor/director.
  • Actively coordinate with other individuals and/or entities essential in the physical and/or behavioral care delivery for the participant.
  • Research and develop alternative solutions to participants' needs.
  • Ensure confidentiality regarding sensitive material and private health information of each individual served.
  • Attend required trainings and recertification classes.
  • Confer regularly with supervisor/director(s) to understand and consider factors such as participants' needs, abilities, learning levels, and physical limitations.

Requirements

  • BS/BA degree in related coursework.
  • Minimum of 3 years related experience providing case management for individuals with Intellectual Disabilities, Autism Spectrum Disorders, or Physical Disabilities.
  • Experience with Microsoft Office and state-maintained databases.
  • Must maintain proficiency in company-sponsored training and certifications.
  • Maintain CPR/First Aid certification and updated state-related clearances.
  • Valid Driver's License including personal vehicle insurance coverage for liability and current vehicle registration.
