About the position

The Targeted Case Manager Supervisor (TCM Supervisor) is responsible for overseeing the performance and compliance of assigned Targeted Case Managers. This role includes providing administrative oversight, clinical consultation, and ensuring adherence to contractual obligations while delivering quality care to clients. The TCM Supervisor will also engage in quality assurance processes and maintain effective communication with clients and providers.

Responsibilities

  • Monitors contractual compliance and provides administrative oversight to assigned Targeted Case Managers.
  • Reviews notes and documentation submitted by Targeted Case Managers.
  • Performs random chart audits as part of the departmental quality assurance process.
  • Monitors transition planning and discharges to ensure compliance with contracts.
  • Conducts internal satisfaction surveys by contacting families served by the TCM program and maintains records.
  • Ensures TCMs follow incident reporting procedures compliant with Federal and State Law and submits incident reports to the QA Department.
  • Provides customer service to providers and clients by making appropriate interventions and documenting such interventions.
  • Provides consultation to direct providers to ensure proper client service provision.
  • Follows HIPAA and confidentiality rules.
  • Provides direct care services to clients as assigned.
  • Attends staff and management meetings as required by the Agency.
  • Participates in staff development trainings and workshops.
  • Enters data as required in the electronic database system.
  • Attends meetings and trainings in the community.
  • Generates correspondence as required.

Requirements

  • Bachelor's Degree in Social Work or related field with at least 1 year of experience with Medicaid.
  • Master's Degree preferred.
  • Bilingual (Spanish and/or Creole) preferred.
  • CBHCMS Certification Mandatory.
  • Willing to obtain a Medicaid provider if not currently active.

  • Knowledge of Medicaid regulations and compliance standards.
