About the position

The TASC Care Manager position at Coastal Horizons Center is a full-time role focused on providing care management services to justice-involved individuals in Wake County, NC. The primary purpose of this role is to administer psycho-social screenings, develop individualized service plans, and coordinate access to various support services, ensuring clients receive the necessary care and advocacy within the justice and behavioral health systems.

Responsibilities

• Administer psycho-social screenings/assessments to gather information on client's backgrounds, needs, and challenges.
• Develop individualized service plans in collaboration with clients to address specific goals and objectives.
• Coordinate linkage to behavioral health treatment and medical, educational, and vocational services as needed.
• Advocate for clients' rights and access to services within the justice and behavioral health systems.
• Regularly monitor clients' progress and compliance with recommended treatment services.
• Prepare and submit detailed progress reports to relevant stakeholders, including probation officers.
• Foster a supportive and trusting relationship with clients to encourage their active participation in the care management process.
• Provide ongoing professional care management services for the justice-involved population.

Requirements

• High school diploma or higher.
• Minimum of two (2) years of experience working in the justice and/or behavioral health field.
• Knowledge of recovery, harm reduction, and wellness-based concepts.
• Strong communication and interpersonal skills.
• Ability to work collaboratively with diverse stakeholders.
• Familiarity with local community resources and support services.
• Empathy and cultural competence in working with individuals from diverse backgrounds.
• Proficient computer skills, including navigating the internet, word processing and spreadsheet applications, and primary email platforms.

Nice-to-haves

• Bilingual Spanish/English speaking skills are a plus!
• Preference for individuals registered with the North Carolina Addiction Specialist Professional Practice Board (NCASPPB) and/or individuals with North Carolina Peer Support Specialist certification.
