About the position

The Integrated Team Counselor (ITC) position within the Adult Community Clinical Services (ACCS) Integrated Teams is a vital role that focuses on providing clinical interventions and peer and family support to individuals in the community. The ITC will work under the guidance of the Team Leader to facilitate engagement, support functioning, and maximize symptom stabilization and self-management for individuals. The primary goal is to assist individuals in developing skills, establishing natural supports, and accessing resources necessary for successful community living. In this role, the ITC will be responsible for delivering services and interventions as outlined in the Treatment Plan. This includes providing education, supervision, and direct support to help individuals maintain their physical and emotional health, live safely in the community, and achieve their personal goals. The ITC will engage with individuals in a manner that recognizes their uniqueness and promotes mutual learning and growth. Services will be delivered in alignment with the Division Mission Statement, program policies, and the standards set by Riverside and the Department of Mental Health. Key responsibilities include contributing to person-centered assessments and service planning, advocating for individuals, coordinating with other services, and providing rehabilitation interventions such as skills training in community settings. The ITC will also educate and train individuals in essential life skills, assist with accessing healthcare, maintain clinical records, and ensure compliance with medication administration procedures. This position requires a commitment to fostering positive relationships and supporting individuals in navigating conflicts effectively.

Responsibilities

  • Provide input to person-centered assessment and service planning.
  • Advocate, coordinate, and link individuals with other services and the community.
  • Deliver rehabilitation interventions, including skills training in community environments.
  • Educate and train individuals in life skills such as budgeting, banking, and personal hygiene.
  • Model and teach skills for developing positive relationships and conflict negotiation.
  • Assist individuals in accessing necessary and appropriate healthcare.
  • Maintain clinical records in accordance with DMH, agency, and division policies and procedures.
  • Perform medication administration and documentation procedures in compliance with relevant policies and regulations.

Requirements

  • Demonstrated ability to work effectively in a team environment.
  • Excellent written and verbal communication skills in English.
  • Valid driver's license and dependable transportation.
