About the position

The position involves planning, directing, and coordinating activities of a nursing unit at Barnes-Jewish Hospital, under the direction of a manager. The role includes performing direct patient care, serving as a clinical resource for nursing and support staff, and ensuring the nursing process is effectively assessed, planned, evaluated, and documented. The position is integral to the multidisciplinary team approach in providing high-quality care to patients, particularly those undergoing kidney and liver transplants.

Responsibilities

• Facilitates identification and resolution of gaps in care from hospitalization, at discharge, and into the ambulatory care setting.
• Critically assess patient data to proactively mitigate impending changes in condition and establishes consults to interprofessional team as appropriate.
• Leads the development and implementation of complex individual plans of care with defined goals in collaboration with other members of the interprofessional team and the patient, family, or caregiver.
• Actively utilizes and champions scientific inquiry for evidence-based practices, participates in journal club, and facilitates practice change based on new knowledge.
• Explores opportunities to advance nursing practice and support community service health events.
• Analyzes gap in practice standards and provides feedback to peers in collaboration with leadership to enhance safe patient care.
• Leads the evaluation of the patient and family experience, and adjusts care provided to improve patient satisfaction and quality of care.

Requirements

• Nursing Diploma or Associate's Degree in Nursing
• 2-5 years of nursing experience
• Registered Nurse (RN) license

Nice-to-haves

• Bachelor's Degree in Nursing
• 5-10 years of nursing experience
