About the position

The Team Manager is responsible for leading the Patient Care Team, supervising and coordinating interdisciplinary team members to ensure continuity of care from admission to discharge or transfer. This role involves patient advocacy, maintaining patient records, and overseeing all patient care services, including volunteer and bereavement support. The Team Manager also collaborates with physicians on palliative care measures and manages team schedules to provide 24/7 patient services.

Responsibilities

  • Supervise, evaluate, and coordinate the interdisciplinary team members.
  • Ensure continuity of care from admission to discharge or transfer.
  • Serve as patient advocate and coordinator for social service and health care providers.
  • Maintain patient records from intake through discharge or transfer.
  • Direct all patient care services for the team, including volunteer and bereavement services.
  • Review pending patient admissions and new admissions to ensure eligibility and appropriateness.
  • Collaborate with team and attending physicians regarding palliative care measures.
  • Monitor team services provided to ensure they meet patient/family needs.
  • Manage the team schedule and coordinate services 24/7.
  • Facilitate team meetings to update and revise care plans based on patient needs.
  • Participate in on-call rotation.
  • Interview, select, train, supervise, and evaluate team staff.
  • Ensure staff competence and performance through field visits and evaluations.
  • Oversee staff and volunteer schedules to distribute workload equitably.
  • Review and approve payroll for accurate documentation of time and expenses.
  • Monitor clinical records and team activities for compliance with standards.
  • Develop and implement performance improvement activities.
  • Address customer service issues and analyze causes to improve performance.
  • Verify and approve admitting diagnoses, treatments, staffing, and supplies.
  • Monitor resource utilization to ensure cost-effective service delivery.
  • Attend inservices, educational seminars, and workshops for professional development.

Requirements

  • Reliable transportation with appropriate license and insurance coverage.
  • Knowledge of primary medical and nursing care principles, especially in oncologic nursing and palliative care.
  • Understanding of interpersonal dynamics within an interdisciplinary health care team.
  • Working knowledge of VITAS information system (Vx) and management reports.
  • Two years of successful supervisory experience in a health care organization.
  • Current and valid R.N. License to practice in the state.
