About the position

The Team Manager at VITAS Healthcare is responsible for leading the Patient Care Team, ensuring the delivery of high-quality care to patients from admission through discharge or transfer. This role involves supervising and coordinating the interdisciplinary team, advocating for patients, and maintaining accurate patient records. The Team Manager also oversees patient care services, staff supervision, quality improvement, and customer service, while managing resources effectively to meet patient needs and organizational goals.

Responsibilities

  • Supervise, evaluate, and coordinate the interdisciplinary team members.
  • Ensure continuity of care from admission to discharge or transfer to bereavement.
  • Serve as a patient advocate and coordinator for social service and health care providers.
  • Maintain patient records from intake through discharge or transfer to bereavement.
  • Direct all patient care services for the team, including volunteer and bereavement services.
  • Review pending patient admissions and ensure eligibility and appropriateness criteria are met.
  • Collaborate with physicians regarding palliative care measures for pain and symptom management.
  • Monitor team services provided to ensure they meet patient and family needs.
  • Manage the team schedule and coordinate services 24/7.
  • Facilitate team meetings to update and revise care plans based on patient needs.
  • Participate in on-call rotation for patient care support.
  • Interview, select, train, supervise, and evaluate team staff.
  • Ensure staff competence through field visits and evaluations.
  • Oversee staff schedules and territory assignments for equitable workload distribution.
  • Review and approve payroll for accurate documentation of time and expenses.
  • Monitor clinical records to ensure compliance with VITAS policy and regulatory standards.
  • Develop and implement performance improvement activities.
  • Address customer service issues and grievances promptly and satisfactorily.
  • Analyze customer service issues to identify causes and improve performance.
  • Visit facilities to ensure care plan integration and customer satisfaction.
  • Participate in professional and community service organizations.
  • Verify and approve patient care resources and costs.
  • Attend inservices and educational seminars for professional development.

Requirements

  • Current and valid RN License to practice in the state of Florida.
  • Bachelor's degree preferred.
  • Two years of successful supervisory experience in a healthcare organization.
  • Knowledge of primary medical and nursing care principles, particularly in oncologic nursing and palliative care.
  • Understanding of interpersonal dynamics within an interdisciplinary healthcare team.
  • Familiarity with VITAS information system (Vx) and management reports.
  • Reliable transportation with appropriate license and insurance coverage.

Nice-to-haves

  • Experience in hospice or end-of-life care settings.
  • Certification in palliative care or hospice management.
  • Strong organizational and management reporting skills.
