About the position

The Care Manager, Telephonic Behavioral Health 2, plays a crucial role in assessing and evaluating the needs of members in a telephonic environment to help them achieve and maintain optimal wellness. This position involves guiding members and their families towards appropriate resources for care and wellbeing, employing various strategies to manage physical, environmental, and psycho-social health issues. The Care Manager is responsible for monitoring patient care, creating care plans, and ensuring that patients progress towards desired outcomes while navigating complex situations with minimal direction.

Responsibilities

  • Assess and evaluate members' needs to achieve optimal wellness.
  • Guide members and families towards appropriate resources for care.
  • Monitor patient care through continuous assessments and evaluations.
  • Create and manage member care plans as needed.
  • Identify and resolve barriers to effective care.
  • Participate in interagency planning meetings to discuss treatment goals and progress.
  • Collaborate with external parties including ABA providers and community agencies.

Requirements

  • Bachelor's degree in Social Work or Behavioral Health related field.
  • 3 - 5 years of experience in Social Work or Behavioral Health.
  • 1 or more years of experience working with the pediatric population.
  • 1 year of managed care experience.
  • Passion for improving consumer experiences.

Nice-to-haves

  • Master's degree in a behavioral health field such as social work or psychology.
  • License in Social Work or Behavioral Health (LMHC, LMFT, LCSW).
  • CCM certification or willingness to obtain within 1 year.
  • Knowledge of community health and social service agencies.
  • Experience in handling complaints and grievances related to member care.
