The Telephonic Care Manager, RN Intern at Humana is a role designed for transitioning military service members or military spouses, focusing on utilizing acquired nursing skills to support Humana's Medicaid member population. The role is primarily remote, requiring effective communication and independent decision-making within established guidelines.

Responsibilities

• Utilize Humana's Medicaid training to assess and evaluate members' needs.
• Engage in strategies to manage members' physical, environmental, and psycho-social health issues.
• Identify and resolve barriers to effective care.
• Continuously monitor patient care through assessments and evaluations.
• Create member care plans as needed.
• Make independent decisions regarding work methods in ambiguous situations.

Requirements

• Transitioning Service Member eligible for SkillBridge or military spouse eligible for the fellowship.
• Active unrestricted Licensed Registered Nurse (RN) in the state of residence or compact state license without disciplinary action.
• Clinical acute care experience in a hospital floor, skilled, or rehabilitation setting.
• Comprehensive knowledge of Microsoft Office Suite and ability to troubleshoot basic technical issues.

Nice-to-haves

• Interest in working in the managed care industry.
• Experience with case management, discharge planning, and patient education for adult acute care.
• Certified Case Manager (CCM) certification.
• Experience with populations with intellectual function and adaptive behavior limitations.
• Residence in the state of Florida.
