About the position

The Temporary Intake Specialist at 24 Hour Home Care plays a crucial role in the operational functions of the Social Recreation service line within the Disability Services Division. This position is focused on driving program growth and ensuring effective service delivery from referral to ongoing support for clients. The specialist will manage a caseload of families, conduct assessments, and develop Individual Service Plans (ISP) while embodying the company's core values and commitment to making a positive impact in the lives of those served.

Responsibilities

• Conduct assessments and develop Individual Service Plans (ISP) for clients.
• Facilitate goal reviews and planning team meetings for direct services.
• Manage a caseload of 100+ families from referral to service initiation.
• Utilize person-centered planning techniques to establish tailored goals for program participants.
• Provide guidance to Social Recreation Direct Support Providers using person-centered methods.
• Coordinate with Regional Center staff and other departments to ensure seamless service delivery.
• Address consumer inquiries and concerns via various communication channels.

Requirements

• Fluent in Spanish.
• Minimum of 3 years experience in person-centered practices and/or developing care plans (strongly preferred).
• Certification or experience in person-centered planning and thinking (preferred).
• Proficient in Microsoft Office: Word, Excel, Outlook.
• Strong communication skills to develop relationships and express ideas clearly.
