About the position

The Therapy Program Manager at HealthPro Heritage is responsible for overseeing rehabilitation services in various settings, ensuring compliance with regulations, and maintaining a clinical caseload. This role emphasizes collaboration, professional growth, and a commitment to delivering high-quality care to residents in Assisted Living, Independent Living, and CCRC settings.

Responsibilities

  • Maintains a clinical caseload.
  • Demonstrates strong clinical knowledge and understands the role of rehabilitation for all clinical disciplines and product lines within the Assisted Living, Independent Living and CCRC settings.
  • Ensures compliance with Federal, State and Local laws, rules and regulations related to therapy service delivery.
  • Assesses data reflective of the resident's status and provides age-specific care according to department policies and procedures.
  • Demonstrates good planning, organizational and interpersonal skills necessary to maintain a safe and goal-directed work environment.
  • Acts as a liaison between HealthPRO-Heritage management, Home Health, Hospice and community administration.
  • Integrates clinical programs and addresses community-specific needs related to marketing.
  • Participates in community-based committees to promote clinical programs and marketing goals.
  • Participates in the implementation of Quality Improvement Programs.
  • Facilitates open communication in patient care meetings while adhering to HIPAA guidelines.
  • Provides education to the community, healthcare providers, and residents regarding therapy services.
  • Participates in community events to promote therapy services.
  • Plans, schedules, coordinates and directs rehabilitation services for the department.
  • Determines staffing needs and schedules to meet resident care demands.
  • Promotes positive employee relations through open communication and problem-solving.

Requirements

  • Must be a licensed Physical Therapist or Physical Therapist Assistant.
  • Minimum of 1 year Outpatient/Home Health Experience.
  • Bachelor of Arts or Science Degree or a Master's Degree.
  • Assumes responsibility for ongoing continuing education and professional development.
  • Must maintain valid state professional licensure.
