About the position

The Rehabilitation Program Manager is responsible for overseeing and managing all aspects of rehabilitation services within an Assisted Living Facility. This role includes maintaining a clinical caseload while ensuring compliance with relevant laws and regulations. The manager serves as an advocate and mentor to the interdisciplinary team, promoting superior clinical and fiscal integrity, and addressing the needs of residents and staff alike.

Responsibilities

  • Maintains a clinical caseload.
  • Demonstrates strong clinical knowledge and understands the role of rehabilitation for all clinical disciplines and product lines within the Assisted Living, Independent Living and CCRC settings.
  • Ensures compliance with Federal, State and Local laws, rules and regulations related to therapy service delivery.
  • Assesses resident's status and provides care according to department policies and procedures.
  • Maintains a safe, orderly and goal-directed work environment for self and the therapy team.
  • Acts as a liaison between HealthPRO-Heritage management, Home Health, Hospice and community administration.
  • Integrates clinical programs and addresses community-specific needs related to marketing.
  • Participates in Quality Improvement Programs and patient care meetings.
  • Provides education to community members, physicians, and staff regarding therapy services.
  • Plans, schedules, coordinates and directs rehabilitation services for the department.
  • Determines staffing needs and schedules to meet resident care demands.
  • Conducts performance appraisals of rehabilitation personnel and oversees documentation audits.
  • Monitors personnel compliance standards and assists in resolving staff and resident-related issues.
  • Develops special programs for resident participation in therapy activities.
  • Provides leadership to and directly manages clinical therapy staff.
  • Ensures a safe environment and utilizes tools and equipment safely.

Requirements

  • Oregon Licensed Physical Therapist.
  • Previous management experience within a similar setting preferred.
  • Associate's Degree in an allied health field, Bachelor of Arts or Science Degree or a Master's Degree.
  • Must maintain valid state professional licensure.
  • Assumes responsibility for ongoing continuing education and professional development.

Nice-to-haves

  • Experience in Assisted Living, Independent Living, and CCRC settings.
  • Knowledge of therapy service delivery regulations.
  • Strong planning, organizational, and interpersonal skills.
