About the position

At NorthBay Health, the Transfer Center Coordinator plays a vital role in the healthcare system by facilitating both inbound and outbound patient transfers. This position is essential for ensuring that patients are transferred efficiently and effectively between various healthcare facilities, including tertiary acute care hospitals. The Transfer Center Coordinator is responsible for coordinating emergent and urgent transfers, as well as consults from NorthBay Health to other facilities. This role requires the establishment of systems that promote cost containment while maintaining high-quality patient care. The Transfer Center Coordinator must possess excellent communication and organizational skills to ensure seamless coordination among different departments, healthcare facilities, and external agencies. Collaboration is key, as this position works closely with all areas of NorthBay Health to facilitate timely and appropriate patient transfers, regardless of the patient's complaint or need. The role also involves liaising with the case management and utilization review teams, as well as the Administrative Coordinator, to manage various aspects of patient transfers, including capitated patients, repatriations, and urgent transfer notifications to health plans. In addition to these responsibilities, the Transfer Center Coordinator is tasked with re-triaging pediatric, trauma, stroke, and STEMI patients. Acting as a liaison between medical staff, nursing staff, pre-hospital personnel, referring centers, and external EMS agencies, the Coordinator ensures that all parties are informed and that patient transfers are executed smoothly and efficiently.

Responsibilities

  • Facilitate and coordinate inbound and outbound patient transfers.
  • Manage tertiary transfers, both emergent and urgent.
  • Coordinate consults from NorthBay Health to tertiary acute care hospitals.
  • Establish systems for cost containment and quality patient care.
  • Collaborate with various departments and healthcare facilities for seamless patient transfer.
  • Liaise with case management and utilization review teams.
  • Handle notification calls to health plans regarding patient transfers.
  • Re-triage pediatric, trauma, stroke, and STEMI patients as needed.
  • Act as a liaison between medical staff, nursing staff, pre-hospital personnel, and external EMS agencies.

Requirements

  • Associate Degree in a healthcare related field required; Bachelor's degree preferred.
  • Minimum two years of related clinical experience.
  • Previous experience coordinating transfer services preferred.
  • Excellent oral and written communication skills.
  • Strong organizational skills and ability to set priorities.
  • Ability to work independently and as an effective team member.
  • Proficiency in word processing and healthcare IT systems.
  • Statistical/data management skills preferred.
  • Ability to work efficiently in a fast-paced and high-pressure environment.

  • Experience with healthcare IT systems and basic computer software.
