About the position

The Transition Coordinator for the Senior Living Division at National Church Residences is responsible for facilitating the transition of seniors into appropriate living arrangements and services. This role involves acting as a liaison between healthcare and housing professionals, ensuring that individuals receive the necessary support and resources tailored to their needs. The coordinator will work closely with case managers and service providers to enhance the quality of life for seniors in the community.

Responsibilities

  • Serve as a liaison between healthcare and housing professionals, including service coordinators and discharge planners.
  • Assist individuals in arranging services based on community resources according to their needs and preferences.
  • Help individuals locate suitable housing and purchase necessary materials and supplies for living.
  • Ensure individuals receive entitled benefits by advocating on their behalf.
  • Provide quality customer service to residents, addressing questions, concerns, and assisting with their needs.

Requirements

  • High school diploma or GED required; Bachelor's Degree in social work or a related field preferred.
  • Minimum of 1 year of experience in social service delivery with elderly and/or family populations; 2-4 years preferred.
  • Demonstrated working knowledge of supportive services and resources in the area served.

Nice-to-haves

  • Experience in home health or hospice care settings.
  • Familiarity with local community resources for seniors.
